A retrospective study compared the safety and efficacy of spironolactone versus bicalutamide in treating female pattern hair loss (FPHL). 120 eligible females aged 18 to 50 with FPHL were categorized into two groups: Group A received 100 mg of spironolactone daily, and Group B received 50 mg of bicalutamide daily alongside topical minoxidil 2%. After approximately 24 weeks of treatment, results showed a statistically significant mean reduction in hair loss severity score of 19.51% in the spironolactone group compared to 28.20% in the bicalutamide group. Additionally, global photographic assessment revealed marked improvement in the bicalutamide group. Despite limitations, the study suggests that bicalutamide may offer greater efficacy and safety in treating FPHL compared to spironolactone.
