This study assessed the outcomes of 310 eyes with small choroidal melanoma (CM) treated using ruthenium-106 (Ru-106) plaque brachytherapy. Tumor recurrence occurred in 17% of cases, with metastatic disease and melanoma-related deaths in 5.8% and 3.9% of patients, respectively, over a median follow-up of 54 months. At 5 and 10 years, recurrence rates were 17% and 26%, while metastasis rates were 4.8% and 16%. Factors predicting recurrence included location, plaque size, and final tumor thickness. Early treatment is crucial for improving survival outcomes in small CMs.
