In this study, the surgical outcomes of detachments related to aggressive retinopathy of prematurity (AROP) were evaluated. AROP is a severe variant of retinopathy of prematurity (ROP) characterized by rapidly forming fibrous tissue, often leading to Stage 5 disease. Data from 43 eyes of 26 babies diagnosed with AROP-related detachments were analyzed, with the majority of eyes having undergone primary interventions like laser or intravitreal bevacizumab injections before surgery. Lens sparing vitrectomy (LSV) was found to yield better visual outcomes, with 58% of eyes exhibiting favorable anatomical outcomes and 53% showing positive functional outcomes. The challenges of AROP include its rapid progression to advanced ROP stages, necessitating vigilant monitoring and multiple interventions.
