The study investigated the mechanism of diuretic resistance in heart failure patients and whether it is related to insufficient furosemide tubular delivery or low tubular responsiveness. Fifty patients were divided into two groups based on previous furosemide use, and diuretic response and urine furosemide concentrations were measured. The study found that the blunted diuretic response in chronic furosemide users is driven by decreased tubular responsiveness rather than insufficient furosemide tubular delivery.
