The clinical relevance of excisional margins after large loop excision of the transformation zone for the persistence of cervical dysplasia

A study investigated the clinical relevance of margins on the presence of cervical intraepithelial neoplasia (CIN) after large loop excision of the transformation zone (LLETZ). The study included 567 women who underwent LLETZ for cervical dysplasia. After a follow-up of 14 months, 9% with affected margins and 4% with clear margins had persistent cervical dysplasia. Positive human papillomavirus (HPV) status was an independent risk factor. Women with affected margins and older age were less likely to have clear HPV. The study suggests that high-risk HPV, excisional margin status, and age can help in risk stratification and individualized management of cervical dysplasia.
