The effect of piezo‐lithotripsy with 2‐and 8‐mm focus sizes on stone disintegration and renal injury

The study aimed to compare the effectiveness and safety of shock-wave lithotripsy (SWL) using narrow-focus and wide-focus for treating renal stones. The double-blind randomized trial involved adult patients with a specific type of kidney stone. The patients were divided into two groups, one receiving narrow-focus SWL and the other receiving wide-focus SWL. The stone-free rate after the first SWL session was 79.2% for the narrow-focus group and 69.1% for the wide-focus group. The levels of urinary markers indicating renal injury were comparable between the two groups. However, the narrow-focus group had higher levels of a specific urinary marker immediately after the procedure. The overall stone-free rates after three sessions were similar for both groups, but the narrow-focus group experienced more pain and higher-grade hematuria. In conclusion, both narrow-focus and wide-focus SWL had similar outcomes and retreatment rates, but narrow-focus SWL resulted in more pain and hematuria.
