The Evolution and Outcomes of the “Maddern Procedure” for the Treatment of Subglottic Stenosis

This study presents a consecutive case series of the first 28 patients who underwent the "Maddern Procedure" for subglottic stenosis. The procedure involves complete resection of subglottic scarring either through a transcervical or transoral approach. The study reports that the procedure was successfully performed in all patients without complications. The use of buccal grafts instead of skin grafts yielded favorable results. The study also found that high stenosis cases showed superior outcomes compared to cases involving the upper trachea. Overall, 73% of the patients had favorable outcomes, and 92% reported that they would undergo the procedure again. The study concludes that the procedure is safe and addresses the recurrent nature of the disease but requires technical expertise.
