This study aimed to develop an algorithm to distinguish patients with bisphosphonate-related osteonecrosis of the jaws (BRONJ) from healthy controls using cone-beam computed tomography (CBCT) images. The algorithm evaluated both trabecular and cortical bone changes throughout the mandibular bone. A patient data set was created from CBCT images of 7 BRONJ patients and 8 healthy controls. The proposed algorithm had several steps, including preparation and background cancellation, mandibular bone segmentation and centerline determination, spatial transformation of gray values, and classification. The algorithm was able to differentiate healthy and diseased bone groups with high accuracy in the present test sample.
