The ribosomal protein S6 kinase alpha-1 (RPS6KA1) induces resistance to venetoclax/azacitidine in acute myeloid leukemia

A recent study has identified a potential mechanism of resistance to the combination therapy of venetoclax and azacitidine in acute myeloid leukemia (AML). The study utilized a genome-wide CRISPR/Cas9 library screen and identified several genes associated with resistance to the treatment. One of the most significant genes found was ribosomal protein S6 kinase A1 (RPS6KA1). When an RPS6KA1 inhibitor, BI-D1870, was added to venetoclax/azacitidine treatment, it reduced cell proliferation and colony-forming potential compared to the combination therapy alone. Moreover, BI-D1870 restored sensitivity to venetoclax/azacitidine in AML cells that had acquired resistance. The study suggests that targeting RPS6KA1 could be a strategy to overcome resistance and improve the effectiveness of the treatment.
