Breast cancer (BC) remains a global health concern, constituting a quarter of all female cancers. Traditional methods involve invasive procedures, but recent studies spotlight extracellular vesicles (EVs) sourced from liquid biopsies as potential game-changers. EVs, instrumental in intercellular communication, carry bioactive cargos mirroring their cell of origin. In the context of cancer, they reshape the tumor microenvironment (TME), influencing cancer progression, including metastasis. Non-coding RNAs (ncRNAs), pivotal in cellular processes, travel within EVs, emerging as liquid biopsy candidates for early BC detection. This review delves into the evolving research on ncRNAs and EVs in BC, emphasizing their roles in cancer progression via bidirectional TME communication and their diagnostic potential.
