Therapeutic Effects of Dual Dopaminergic Modulation With l-DOPA and Chlorpromazine in Idiopathic Cervical

This study explores dual dopaminergic modulation (DDM) using l-DOPA and chlorpromazine (CPZ) to address imbalanced D1 and D2 signals in the striosomal-matrix system, implicated in dystonia. The study enrolled 21 botulinum toxin-resistant idiopathic cervical dystonia (CD) patients; the combination significantly alleviated CD motor symptoms and associated pain in a dose-dependent manner. Sole administration of l-DOPA or CPZ showed no improvement. The findings suggest that DDM with l-DOPA and CPZ could effectively treat CD symptoms, indicating a potential increase in striosomal D1-signaling. Class III evidence establishes the superiority of this combination over individual treatments.
