A comprehensive meta-analysis examined the efficacy of P2Y12 inhibitor monotherapy following a brief dual antiplatelet therapy (DAPT) among patients undergoing percutaneous coronary intervention (PCI). Analyzing 6 trials involving 25,960 patients, the study compared ticagrelor and clopidogrel monotherapy to standard DAPT. Ticagrelor monotherapy was found noninferior to DAPT for major outcomes, including all-cause death, MI, or stroke, with reduced major bleeding and net adverse clinical events (NACE). However, clopidogrel monotherapy, though reducing bleeding, did not meet noninferiority criteria due to higher noncardiovascular death risk. The findings suggest ticagrelor monotherapy's superiority in safety and efficacy over clopidogrel.

A retrospective study of 66 patients with Budd–Chiari syndrome found that liver stiffness measurement (LSM) effectively distinguished controlled from uncontrolled clinical status, with an AUC of 0.985. An LSM cutoff of 25 kPa achieved 97.5% sensitivity and 92.5% specificity. Spleen stiffness provided complementary but inferior diagnostic performance.

A newly developed evidence-based guideline provides recommendations for diagnosing, staging, and managing keratoconus in children and adults. Using systematic evidence synthesis and the GRADE approach, it addresses corneal cross-linking, intrastromal corneal ring segments, keratoplasty, and individualized follow-up. Most recommendations remain conditional because evidence certainty is low or very low in several clinical areas.

UROLOGY

NEUROLOGY

An analysis of 854 women from the European Prevention of Alzheimer’s Dementia cohort found that higher serum estradiol was associated with lower baseline tau levels in cerebrospinal fluid, particularly among APOE ε4 carriers. A nationwide Colombian study identified 4,608 healthcare encounters related to childhood interstitial and diffuse lung disease (chILD) from 2016–2024. Average annual prevalence was 3.32 per 100,000 children, highest among infants and with a secondary rise during adolescence. Reported cases increased annually (IRR 1.022), while declining markedly during the COVID-19 pandemic.
