
This retrospective study aimed to analyze the bacterial distribution in amniotic membrane cultures of women with peripartum fever (PPF) and its impact on perinatal outcomes. The study spanned from 2011 to 2019 and focused on the rates of positive birth cultures for Enterobacteriaceae in women with PPF and the trend of ampicillin resistance. The results showed an increasing prevalence of ampicillin-resistant Enterobacteriaceae. Positive birth cultures were associated with maternal bacteremia and neonatal early-onset sepsis. Prolonged rupture of membranes (ROM) of 18 hours or more increased the risk of Enterobacteriaceae-positive cultures, while intrapartum administration of ampicillin and gentamicin reduced the risk. Adverse maternal and neonatal outcomes were more common in Enterobacteriaceae-positive cultures compared to group B Streptococcus (GBS)-positive cultures. The findings suggest the need to reconsider antibiotic prophylaxis treatment for prolonged ROM in women with PPF.
