
This study evaluates the impact of tumor necrosis factor inhibitors (TNFi) on pain pathways in rheumatoid arthritis (RA) and spondyloarthritis (SpA). Among 100 patients starting TNFi therapy, descending pain modulation, assessed by conditioned pain modulation (CPM), significantly improved over six months, particularly in those achieving low disease activity or remission. However, thermal pain thresholds remained unchanged. The findings suggest TNFi enhances central pain modulation in inflammatory rheumatism, offering potential insights into managing persistent pain in RA and SpA.
