
Transanal total mesorectal excision (TME) shows noninferior 3-year disease-free survival compared to laparoscopic TME in rectal cancer patients. The transanal TME group had 82.1% disease-free survival vs. 79.4% in the laparoscopic group, with a difference of 2.7%. The study supports transanal TME as an effective option for patients with stage I-III rectal cancer, especially in centers lacking robotic expertise. Both methods showed similar local recurrence and overall survival rates.
