Transoral Surgery in HPV‐Positive Oropharyngeal Carcinoma: Oncologic Outcomes in the Veterans Affairs System

A study analyzed 161 patients with HPV-positive oropharyngeal carcinoma who underwent transoral robotic surgery (TORS). The patients were categorized into low-risk, intermediate-risk, and high-risk groups. The study found that TORS followed by adjuvant treatment resulted in 5-year disease-specific survival (DSS) rates of 100%, 90%, and 89% for low, intermediate, and high-risk patients, respectively. Poor DSS was associated with extranodal extension (ENE), advanced tumor stage (pT3/T4), and more than five metastatic lymph nodes. The cumulative incidence of recurrence and distant metastasis varied across risk groups. The study concluded that TORS with adjuvant therapy yielded favorable survival outcomes similar to those reported in academic centers and trials.
