Troponin T Levels More Commonly Elevated Than Troponin I in ED Patients

A cohort of 1,134 Emergency Department patients, showed that 65% had elevated high-sensitivity cardiac troponin T (hs-cTnT), compared to 30% for troponin I (hs-cTnI). A strong correlation (r = 0.86, p < 0.01) was observed between the two biomarkers, with 39% of cases showing elevated hs-cTnT alongside normal hs-cTnI. The area under the curve for detecting acute myocardial infarction (AMI) was 0.80 for hs-cTnT and 0.87 for hs-cTnI. However, hs-cTnT displayed lower specificity (39%) compared to hs-cTnI (80%), resulting in a lower positive predictive value for AMI. Hazard ratios for 1-year mortality were 1.52 for hs-cTnT and 1.26 for hs-cTnI, indicating the clinical importance of both markers in patient prognosis.
