A study explored the correlation between the triglyceride-glucose (TYG) index and mortality risk in non-diabetic septic patients. The study included 2,213 patients with sepsis admitted to the ICU, with 549 (24.8%) dying within 28 days of admission. The analysis revealed a non-linear association between TYG and mortality risk. Higher TYG levels were linked to increased mortality, with a fully adjusted hazard ratio 2.68. TYG demonstrated an area under the curve (AUC) of 67.7%, outperforming triglycerides alone (64.1%), blood glucose (62.4%), and Glasgow Coma Scale (63.6%), and was comparable to the SOFA score (69.3%). No significant interactions were found except for the COPD subgroup (interaction P-values: 0.076–0.548). These findings suggest that TYG is a valuable independent predictor of all-cause mortality in septic patients within 28 days of hospitalization.
