Ulnar Collateral Ligament Injuries in Athletes: Is Surgery Always Necessary?

A systematic review was conducted to determine the rate of return to sports and return to previous level of play in athletes with medial elbow ulnar collateral ligament injuries who were treated nonoperatively. The review included 15 studies with a total of 365 patients. Treatment primarily consisted of platelet-rich plasma injections with physical therapy or physical therapy alone. The overall rate of return to sports was 79.7% and the overall rate of return to the previous level of play was 77.9%. Increasing injury severity was associated with lower rates of return to sports. The use of platelet-rich plasma did not significantly impact return to sports rates. Proximal tears had significantly higher return to sports rates than distal tears.
