A study aimed to assess the clinical usefulness of protein induced by vitamin K absence or antagonist-II (PIVKA-II) as a biomarker for hepatocellular carcinoma (HCC) monitoring after liver transplantation. The study included 46 HCC patients who met the Milan criteria. Pre- and post-transplantation levels of PIVKA-II and alpha-fetoprotein (AFP) were measured, and clinical features were determined. The results showed significant correlations between PIVKA-II expression levels and certain clinicopathological features, such as tumor size and the number of pre-transplant transarterial chemoembolizations. PIVKA-II and AFP levels decreased after transplantation and increased in patients with tumor recurrence. Monitoring PIVKA-II levels could serve as a valuable tool for predicting disease severity and detecting early tumor recurrence after transplantation, complementing AFP and imaging tests.
