In this study on end‐stage head and neck squamous cell carcinoma (HNSCC), 79 patients were analyzed. The study aimed to identify factors influencing prognosis and establish a more effective prognostic index. Patients were categorized based on specific factors and prognostic indicators. Comparisons of overall survival were made between groups starting Best Supportive Care. Moreover, patients with a palliative prognostic index (PPI) ≥6 and neutrophil‐to‐lymphocyte ratio (NLR) ≥10 were contrasted with those having PPI <6 or NLR <10. Results revealed that PPI ≥6 and NLR ≥10 correlated significantly with poorer prognoses (p = 0.01 and p = 0.002, respectively). Median survival for cases with PPI ≥6 and NLR ≥10 was 32 days, while it was 64 days for PPI <6 or NLR <10 (p < 0.001). Therefore, the combination of PPI and NLR serves as a valuable prognostic indicator for predicting the prognosis of patients with end‐stage HNSCC.
