
An 8-year prospective study of 183 women found that the levator hiatus area progressively increased in those with at least one vaginal delivery but decreased in women who delivered exclusively by Cesarean section (CS). Additional vaginal births after a first vaginal delivery did not further enlarge the hiatus during straining (Valsalva), suggesting the first delivery has the greatest long-term impact on pelvic floor anatomy.
