Sepsis involves a dysregulated inflammatory response and lacks targeted treatment. The cholinergic anti-inflammatory pathway (CAP), mediated by the vagus nerve, modulates inflammation and offers therapeutic potential. Emerging CAP-related interventions—like vagus nerve stimulation, α7nAchR-targeting drugs, and acupuncture—may help restore immune balance. Heart rate variability may serve as a non-invasive biomarker. Clinical validation is needed to advance CAP-based precision therapies.
