Value of inferior vena cava collapsibility index as marker of heart failure in COPD exacerbation

The study assessed the utility of ultrasound-measured Inferior Vena Cava (IVC) diameter variability in diagnosing heart failure (HF) during acute exacerbation of chronic obstructive pulmonary disease (AECOPD) in emergency department (ED) patients. Among 401 AECOPD patients, 41.1% were diagnosed with HF. ΔIVC demonstrated 37.4% sensitivity, 89.7% specificity, 70.9% positive predictive value, and 66.7% negative predictive value using a 15% threshold. The area under the ROC curve was 0.71, indicating IVC's effectiveness in ruling out HF during AECOPD presentations. Notably, ΔIVC values did not differ significantly between HF patients with reduced or preserved left ventricular ejection fraction. The results showed that ΔIVC has a good value for ruling out HF in ED patients consulting for AECOPD.
