
Virtual reality (VR) is a novel approach in neurorehabilitation, but its feasibility in the acute phase post-stroke is unclear. This cohort study evaluated consecutive patients admitted to the Essen University Hospital Stroke Unit (March–May 2022). Of 326 patients, 172 were analyzed, and 37 (21.5%) underwent VR-guided therapy for upper extremity deficits. Treatment barriers included neuropsychological limitations (22.9%), physical impairments, comorbidities, and altered consciousness (17.8% each). Sessions lasted a median of 20 minutes, with high patient motivation (76%) and therapist feasibility ratings (65%). VR shows promise for enhancing recovery in selected acute stroke patients, warranting further randomized trials.
