
The study evaluated the association between various anthropometric measures and hormonal disturbances in 129 women with PCOS. Results indicated that waist-to-height ratio (WHtR) had the strongest correlation with hyperandrogenism, outperforming body mass index (BMI) and other fat distribution indices. Linear regression models adjusted for age, menstruation status, and insulin resistance showed positive associations between WHtR, BMI, fat mass, and the free androgen index (FAI), while negative associations were observed with sex hormone-binding globulin (SHBG). The WHtR demonstrated the highest area under the curve (AUC = 0.676, P = 0.001) for predicting hyperandrogenism, with a sensitivity of 87.3% and specificity of 39.7%. These findings suggest that WHtR is a more reliable indicator of hormonal imbalances in PCOS women than other obesity measures.
