A study comparing fosfomycin and flumarin as alternatives for antimicrobial prophylaxis in transrectal ultrasound (TRUS)-guided prostate biopsy found that, of 2,900 patients, 11.5% received fosfomycin and 88.5% received flumarin. Fosfomycin showed a non-significant 3% lower rate of infectious complications than flumarin (5.7% vs. 8.6%). Complication risk factors included recent general anaesthesia (OR: 2.216) and prior antimicrobial use (OR: 1.457). Despite the non-significant difference, fosfomycin's ease of administration and low adverse effects suggest it could be an attractive alternative for TRUS-guided prostate biopsy prophylaxis.
