A New Insight into Heart Failure with Preserved Ejection Fraction: The Impact of Exercise on Pulmonary Hypertension

The study aimed to investigate disproportionate exercise-induced pulmonary hypertension (DEi-PH) and its impact on the prognosis of patients with heart failure with preserved ejection fraction (HFpEF). DEi-PH is defined as a mean pulmonary artery pressure-cardiac output relationship (mPAP/CO slope) > 5.2 mmHg/L/min. The study involved 345 patients, including 166 HFpEF and 179 controls, who underwent ergometry exercise stress echocardiography with simultaneous expired gas analysis. HFpEF patients with DEi-PH had worse exercise capacity, depressed right ventricular systolic function, impaired RV-pulmonary artery coupling, limitation in CO augmentation, more right-sided congestion, and worse ventilatory efficiency during peak exercise than those without DEi-PH or controls. HFpEF patients with DEi-PH also had higher rates of composite outcomes of all-cause mortality or heart failure events. These findings suggest that DEi-PH is a pathophysiologic phenotype of HFpEF and reinforce the importance of exercise stress echocardiography for the detailed characterization of HFpEF.
