A Pathology-based Case Series of Influenza- and COVID-19–associated Pulmonary Aspergillosis: The Proof Is in the Tissue

In severe COVID-19 and influenza, invasive pulmonary aspergillosis (IPA) is a frequent coinfection, with its invasiveness debated. Researchers studied histology of ICU patients who died from both viruses, finding 6 proven cases of IPA in each group. Missed diagnoses occurred in 8% of proven cases, but were often confirmation of probable pre-mortem diagnoses (52%). Bronchoalveolar lavage galactomannan testing was sensitive for IPA. Both viruses exhibited similar invasive patterns of IPA, requiring heightened VAPA awareness and mycological bronchoscopic investigation.
