
A study involving 41,677 hemodialysis patients across 21 countries from 2005 to 2018 assessed the impact of active vitamin D prescription on fracture risk. Results showed that 55% of patients received active vitamin D, but it did not significantly reduce the risk of any fractures (hazard ratio 1.02) or hip fractures (hazard ratio 1.00). These findings suggest that active vitamin D may not independently enhance bone strength beyond its role in parathyroid hormone control. The study supports the current KDIGO guideline recommending active vitamin D primarily for individuals with elevated or rising PTH levels
