Advancements in Understanding Vascular Heterogeneity in Bone Marrow Microenvironment

The bone marrow (BM) vasculature plays a central role in hematopoiesis, providing essential support for blood cell development across different stages of life. Recent advances in single-cell technologies and microscopy have revealed the remarkable heterogeneity of BM endothelial cells, which previously were understood only for their paracrine functions. This review discusses how endothelial cell diversity is not just structural but also functional, with distinct endothelial states identified in various BM locations. Additionally, the molecular basis for this vascular heterogeneity and its impact on hematopoiesis in health and disease, especially in leukemia, is explored. By synthesizing the latest research, we address critical questions in the field and outline future directions for clinical applications aimed at improving treatments and understanding the dynamic BM microenvironment.
