Antegrade versus Retrograde Approach for Recanalization of Ostial or Stumpless Coronary Chronic Total Occlusion

This study evaluated percutaneous coronary intervention (PCI) outcomes for ostial or stumpless chronic total occlusion (CTO). The antegrade-only approach had higher success rates than the retrograde (RA) approach, with lower J-CTO scores and more microchannels. RA group had more in-hospital major adverse cardiac events (MACE) and myocardial infarction (MI). A J-CTO score below 2 and microchannels at the proximal stump predicted successful antegrade-only approach PCI. The study suggests that the antegrade-only approach is preferable for less complex CTO lesions and reduces the risk of in-hospital MACE.
