Antimicrobial Overload: Protecting the Patient in ECMO Treatment

A recent study evaluated the pharmacokinetics of antimicrobial drugs in critically ill adult patients receiving extracorporeal membrane oxygenation (ECMO) and found large variations in all PK parameters for all drugs, with only 56% of concentration profiles achieving the target concentration. However, the study had some methodological issues, including small sample size and a lack of a control group, making it unclear if ECMO impacts antimicrobial exposure or if the chosen target concentrations are relevant in this setting. Additionally, the study relied on European Committee on Antimicrobial Susceptibility Testing (EUCAST), Epidemiological cut-off value (ECOFF) or EUCAST breakpoint, which may not take into account the specific situation in its specific environment. More studies are needed to assess the real impact of ECMO support on pharmacological drugs, and therapeutic drug monitoring should be analyzed in accordance with contextual minimum inhibitory concentration (MIC).
