Apalutamide Improves Outcomes in High-Risk Prostate Cancer

The study evaluated neoadjuvant therapy (NAT) outcomes in high-risk prostate cancer (HRPC) patients undergoing radical prostatectomy (RP) and pelvic lymph node dissection (PLND). ADT plus apalutamide showed superior rates of pathological complete response (pCR) or minimal residual disease (MRD) and extended biochemical recurrence (BCR) duration compared to ADT plus abiraterone. Post-intervention PSA levels and AR axis mutations significantly influenced outcomes, emphasizing the need for genetic testing and PSA monitoring in HRPC treatment.
