This study aimed to evaluate and compare the treatment outcomes of root-filled teeth with apical periodontitis treated either non-surgically or by endodontic microsurgery and to assess the influence of clinical and demographic factors. The study analyzed clinical and radiological data from 1157 patients with apical periodontitis in previously root-filled teeth treated between September 2010 and January 2020. The results showed that endodontic microsurgery had a significantly better success rate than non-surgical retreatment, and the success rate was influenced by the tooth type, age of the patient, and preoperative PAI score. Teeth with posts treated non-surgically tended to have better outcomes than those treated with surgery.
