Are haematopoietic stem cell transplants stem cell transplants, is there a threshold dose of CD34-positive cells and how many are needed for rapid posttransplant granulocyte recovery?

A large dataset analysis questions the accuracy of CD34-positive cell numbers as a measure of haematopoietic stem cells (HSCs) after transplant. Human HSC identification lacks precision, complicating recovery predictions. The contribution of various haematopoietic cells to post-transplant marrow function remains uncertain. Moreover, a threshold dose for successful recovery seems biologically implausible. Using umbilical cord blood data, researchers reveal no threshold, and HSC dosage correlated with recipient blood volume, not body weight. These findings have significant transplant practice implications, broadening eligibility and easing CD34-positive cell procurement pressures.
