The retrospective study examined the association between Aspergillus hypersensitivity (AH) and mucus plugging in COPD patients. Among 378 COPD patients, 29 (7.7%) had AH. Patients with AH showed worse lung function (lower FEV1%pred, FEV1/FVC), higher eosinophil counts, and greater prevalence of mucus plugging on chest CT (58.6% vs 31.0%). Multivariate analyses identified Af sIgE levels >0.70 KU/L and higher neutrophil counts as associated with mucus plugging, suggesting a potential link between Aspergillus hypersensitivity and poor COPD outcomes.
