
The study investigated how variations in cytochrome P450 (CYP) 3A422 and cytochrome P450 oxidoreductase (POR)28 affect the pharmacokinetics of tacrolimus in renal transplant recipients. Analyzing data from 18 publications involving 2931 patients, it found that CYP3A422 carriers required lower weight-adjusted daily doses but had higher dose-adjusted trough concentrations compared to CYP3A41/1 carriers, particularly in Caucasians. Conversely, POR28 carriers, especially in CYP3A5 expressers, exhibited higher dose-adjusted trough concentrations. Further studies are needed to validate these findings due to existing limitations.
