This study compared the changes in body weight, cardiometabolic risk factors, and the incidence of type 2 diabetes between individuals with metabolically healthy overweight/obesity (MHO) and metabolically unhealthy overweight/obesity (MUO) during a lifestyle intervention. The study included 1012 participants with MHO and 1153 participants with MUO. The results showed no significant differences in weight loss between the two groups over the 156-week study period. However, participants with MUO experienced greater improvements in certain cardiometabolic risk factors during the low-energy diet phase, while those with MHO had greater reductions in these risk factors during long-term weight maintenance. Furthermore, individuals with MHO had a lower incidence of type 2 diabetes than those with MUO.
