
The study examined the relationship between the blood urea nitrogen/creatinine ratio (BCR) and in-hospital mortality in ICU patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD). Using data from 3399 eligible patients, high-BCR and low-BCR groups were defined by a ROC curve cutoff value of 22.78. Propensity score matching (PSM) balanced baseline characteristics between groups, resulting in 1174 matched pairs. Multivariate logistic regression analysis demonstrated a significantly higher in-hospital mortality rate in the high-BCR group compared to the low-BCR group in both unmatched and matched cohorts after adjustments for confounding factors. Decision curve analysis (DCA) confirmed the clinical utility of the models.
