Best Practices for Differential Diagnosis of Suspected COPD Exacerbations in Acute Care Settings

Patients with chronic obstructive pulmonary disease (COPD) often experience acute exacerbations, which can worsen dyspnea and increase the risk of hospitalization. These exacerbations can be caused by various comorbidities that may mimic or aggravate them. Around 70% of readmissions after an ECOPD hospitalization result from decompensation of other morbidities. Therefore, carefully searching for these comorbidities can help detect them and allow appropriate treatment. A thorough clinical evaluation supplemented by appropriate clinical investigations can guide the healthcare provider to make a precise diagnosis. This perspective provides a practical approach to patients with COPD complaining of worsening respiratory symptoms, particularly dyspnea, to improve outcomes and reduce the personal and societal cost of ECOPDs.
