This study assessed brain development in living fetuses with Down syndrome (DS) using fetal brain magnetic resonance imaging (MRI) biometric measurements. The study examined 10 MRIs of DS-confirmed fetuses and 12 control fetal MRIs with no anomalies. Analysis of fetal brain MRIs using 14 parameters found that DS fetuses had significantly smaller cerebellar vermis height and anteroposterior diameter, while their fourth ventricle was notably larger. Additionally, raw MRIs showed larger right lateral ventricles in DS fetuses. A new parameter, the cerebellar-to-fourth-ventricle ratio, emerged as a strong predictor, distinguishing DS fetuses from controls
