In a study of 4,609 type 2 diabetes patients from the ACCORD trial, significant body fat variation rates were linked to an increased risk of diabetic nephropathy over five years. Participants with high body fat variation had a 13% higher risk of developing diabetic nephropathy (HR 1.13, 95% CI 1.01-1.26) compared to those with low variation. This association was particularly evident in obese individuals (BMI > 30), where the risk increased by 34% (HR 1.34, 95% CI 1.08-1.66). No significant association was found in non-obese individuals. These findings suggest that gradual weight loss strategies should be emphasized for obese diabetic patients to minimize the risk of nephropathy caused by large body fat fluctuations.
