
Children with preschool wheezing represent a heterogeneous population with wide variability regarding their clinical, inflammatory, obstructive, and remodeling patterns. The study hypothesized that assessing bronchial remodeling would help clinicians to better characterize severe preschool wheezers. The study aimed to identify different types of severe preschool wheezers based on bronchial remodeling parameters and to compare their clinical and biological data. A two-class model was identified, and Class BR1 was associated with a higher risk of frequent and severe exacerbations. The study suggests that assessing bronchial remodeling could help identify preschool wheezers at risk of future exacerbations with a favorable benefit-to-risk ratio. The fiberoptic bronchoscopy procedures used were safe and well-tolerated.
