
CABG outperforms PCI in complex coronary cases, regardless of diabetes. This FAME 3 trial subgroup analysis (n=1500, 28.5% with diabetes) found PCI had a higher MACCE risk than CABG (HR: 1.44 in diabetes, 1.50 without). The SYNTAX score influenced outcomes, with CABG superior in intermediate-to-high-risk patients. Findings suggest CABG remains preferable for multivessel disease, particularly in complex cases, irrespective of diabetes status.
