Can we make biologic therapy more affordable?

Monoclonal antibody therapies were recently approved for the treatment of chronic rhinosinusitis with polyposis, which are a new promising treatment and life‐changing for selected patients. Because of their high cost, this new treatment is not affordable for many. The current treatment protocol for dupilumab is bi-weekly injections and re‐evaluation after 16 weeks. To cut costs and decrease the financial burden on the public and private health care systems, we suggest a de‐escalation strategy, hence, a trial of dupilumab once monthly between the weeks 16‐24 for well‐controlled CRSwNP and other co‐morbidities, such as Asthma. Moreover, in case of any worsening of symptoms, q2 treatment protocol can be initiated immediately. This small act can potentially cut costs in half and eventually, offer monoclonal antibody treatment for more patients.
