
Immunotherapies have transformed cancer treatment but carry significant risks of immune-related adverse events (irAEs), particularly cardiovascular toxicities. This scientific statement outlines the cardiovascular effects of immune checkpoint inhibitors (ICIs), CAR-T cells, NK cells, and tumor-infiltrating lymphocytes (TILs), noting complications from severe myocarditis with cardiogenic shock to milder forms of myocarditis, pericarditis, arrhythmias, and non-inflammatory heart failure. Cardiovascular irAEs reflect varied immune responses and are challenging to predict, diagnose, and manage, with underlying mechanisms largely unclear. This review emphasizes the need for further studies on irAE pathophysiology to improve safety in immunotherapy applications.
