
The PROPHY-VAP trial investigated the impact of a single ceftriaxone dose on early ventilator-associated pneumonia (VAP) in severe brain injury patients requiring mechanical ventilation. Conducted across nine intensive care units in French university hospitals, 345 comatose adult patients received ceftriaxone or placebo post-tracheal intubation. Results revealed lower early VAP incidence in the ceftriaxone group (14% vs 32% placebo), with no microbiological differences or adverse effects. The study recommends ceftriaxone inclusion in VAP prevention bundles for brain injury patients on mechanical ventilation. These findings offer crucial insights into antibiotic prophylaxis strategies for acute brain injury patients.
