The review discusses molecular and physiological mechanisms of insulin resistance, focusing on hyperinsulinaemia and its implications. It highlights the challenges and prospects for selective insulin sensitization therapies, such as inhibiting FOXO1 or activating PPARγ. Future therapies could modify the disease course and prevent progression.
