Chasing the Culprit: Enterococcus Faecalis' Role in Treatment-Resistant Apical Periodontitis

The study demonstrates the role of Enterococcus faecalis in refractory apical periodontitis (RAP), a persistent oral infection that causes inflammation, bone destruction, and delayed healing. E. faecalis is the dominant pathogen involved in RAP and has developed multiple strategies to ensure its survival, causing persistent infections. The article reviews the pathogenesis of RAP, including factors such as microorganism immunogenicity, host immunity and inflammation, and tissue destruction and repair. The authors searched PubMed and Web of Science for relevant publications and identified various virulence mechanisms employed by E. faecalis, which modulate the macrophage and osteoblast responses, leading to delayed tissue healing. Understanding these host cell responses will be crucial in designing potential therapeutic strategies to overcome sustained infection and delayed tissue healing in RAP.
